YOUR FIRST VISIT
Your first visit will include a comprehensive evaluation and discussion. The meeting may take anywhere between 60 and 90 minutes, due to the complicated and detailed nature of the meeting. If it saves time, we invite you to complete the following information forms before visiting.
WHAT TO BRING
Bring any pre-completed forms and your doctor’s referral form (if you are given one). If you have had an imaging (a CT or MRI) scan related to your consultation, we request that you bring a CD of the images with you to your visit. The radiology facility performing the service can provide you with this. Please bring any pertinent medical records from your referring doctor and a list of your current medications and dosages.
We are located in the beautiful Temecula area of Southern California. We aim to provide a valuable service to the community of Temecula and the surrounding area. In our state-of-the-art facility, you will be assisted by our unsurpassed staff every step of the way.
AFTER THE VISIT
We will summarize what was discussed during your time with us, and set up a return appointment if necessary. We will keep in close contact with you regarding any planned procedure or surgery. If you have any questions after you leave the office, we are always available to discuss further.
