During your initial visit, we will evaluate your situation and discuss with you what you can expect out of your procedure. The discussion will usually take between 60 and 90 minutes, so we can ensure you are fully informed as to the specifics of your operation. To save time, please print out the New Patient Information Forms below.

We also request that you bring any doctor’s referral forms you may have, as well as a list of your current medications and dosages and any other pertinent medical records from your referring doctor. If you have had an imaging related to your consultation, please bring a CD of the images with you to your visit.
